Abstract :

The patients with oblique hernia with normal musculature are not an indication for the operation. These patients should be treated with high ligation of the sac and closure of the internal ring with the least interference with the physiology of the canal provided that, this physiology is normal. Pre-operative preperation of the patient with large scrotal hernia is essential. Two or three days before surgery the skin of a scrotal hernia including the groin area should be shaved and cleaned with soap and water 2-3 times a day and painted with betadine solution. This was found to reduce the chance of wound infection especially when the patient has a neglected hernia with dry excoriated skin. General anesthesia or spinal can be used; however, local anesthetic is preferable, especially in the old aged. The local anaesthesia used in most patients is by the local infiltration of 1 percent xylocaine. The patient is usually allowed to choose the type of anesthesia, but in general, it is indicated for elderly patients who are sixty years or more. Any young patient who suffers from chest or heart condition is also an indication for local anesthesia. Young patients especially neurotics are not suitable for local anesthesia and should be given spinal or general anesthesia. Before the operation the patient is instructed to empty his bladder. This will avoid its injury during the repair of the floor of the canal.
