Cholesteatoma is not an uncommon pathology that affects the ear. Masses of keratin invade and expand into the ear destroying the surrounding structures by mechanical expansion as well as through proteolytic enzymes that attack bone matrix. Cholesteatoma acquires a different spectrum when it invades medially into the petrous temporal bone. In this rare incidence, medially growing cholesteatoma not only becomes more dangerous but its surgical removal becomes a surgical challenge. Its medial location is surrounded by the brain, cranial nerves and major blood vessels. In this communication we present the different classifications adopted in the literature which serve in the diagnosis and in selecting the appropriate surgical management plan. We also present our surgical experience in managing this rare pathology.
Naguib, a graduate of Ain Shams University Medical School, Cairo Egypt has obtained a Masters degree and a Doctorate degree in Otorhinolaryngology. He accomplished his dissertation in Baylor College of Medicine, Houston, Texas in 1990. Naguib completed a one year clinical fellowship in otology, neurotology and skull base surgery at the Gruppo Otologico, Piacenza, Italy in 1993. He joined the faculty at the Suez Canal University in 1995. In 2005 he was nominated the Professor of Otolarhinolaryngology. Dr. Naguib research is focused on otologic surgery, the facial nerve and skull base surgery as well as balance disorders. Dr. Naguib has received the Best Research Award from the European Federation of Otolaryngologic Societies in 1992. Dr Naguib is a member of the European Skull Base Society and is a corresponding member of the American Academy of Otolaryngology.
